Izek was the right-hand man of the Burgomaster of Valleki, Baron Vargas Vallakovich. He had an enlarged right arm with barbed spines, elongated fingers, and long nails. He lived with the Baron and his wife in their stately manor, and hung around the town square. The people of Valleki feared him, in part due to his ability to produce flame from his enlarged hand, which he claimed was a gift that he awoke with after losing his original arm.

He was obsessed with a woman that he saw in his dreams, who looked strikingly like Ireena. He ordered many dolls from Blinsky's toy shop, and took great care of them. Gale and Dren stole the dolls in order to lure him to the bog, where we killed him, hoping that his death would be no more suspicious than the other disappearances from the Baron's household.

Using one of his dolls and a bloody shirt, Percy framed Fiona Wachter for Izek's death.

After feeling immense guilt for his role in his death, Bellerand resurrected Izek using his gift from the Amber Temple. He promised Izek that he would introduce him to Ireena in exchange for not seeking retribution for his death, but now that Ireena has been taken by Strahd von Zarovich, he has informed Bell that their agreement is no longer valid. Bell used Geas on him to compel him to "remain in Barovia town and help its people to the best of [his] ability."
